Include a default name and required scopes in the URL to generate the personal access token.

How Has This Been Tested?
I haven't tested this via the code (not sure how to build it), but you can see it working by visiting
https://gitlab.com/-/profile/personal_access_tokens?name=GLab&scopes=api,write_repository
I just found out about GLab and added the feature to support passing in the name and scopes to the PAT page on GitLab. When setting it up I noticed they weren't present and could simplify the set up for other new users.
