Add a dedicated second Z trigger binding to both the generic USB input
plugin and the GCA plugin, allowing users to map an additional button
to Z. Also remove the add-binding (+) button from the generic plugin
in favor of click-to-set, reorganize the GCA dialog with grouped button
sections, replace slider group boxes with inline labels, and remove the
unused C-stick threshold slider from the GCA UI.

Add a calibration/test screen for raphnet adapters that displays
real-time N64 controller state via hidapi. Shows all 14 buttons as
color indicators and raw X/Y axis values with progress bars. Only
accessible when raphnetraw is the active input plugin and no ROM is
running, to avoid interfering with game input.

Add a "Stick Range Readout" section to both the generic USB input plugin
and GCA plugin config dialogs showing real-time computed N64 X/Y axis
values with progress bars. The readout reflects the actual values sent
to the game after deadzone and range/sensitivity scaling.

For the generic plugin, the controller image now displays the stick
position using the same processed values, matching the readout exactly.
Input reading supports all mapping types including keyboard bindings.
