Actual behavior

UnauthorizedAccessException - but username is local Administrator

Per the documentation, the UnauthorizedAccessException means

The named mutex exists and has access control security, but the user does not have FullControl.

It sounds like a problem in the system you are running on, not something that can be helped from PSReadLine side.
You may want to upgrade to the latest v2.4.5 PSReadLine and give that a try. See https://github.com/PowerShell/PSReadLine?tab=readme-ov-file#installation-and-upgrading for instructions.
